MAKING A PURCHASE WILL NOT INCREASE YOUR CHANCES OF WINNING.
1. ENTRY: The “Skin Care Foundation Fun in the Sun 101” Sweepstakes (the “Promotion”) begins at 12:01 PM Eastern Time (“ET”) on 04/03/09 and ends at 11:59 PM on 05/08/09 (the “Promotion Period”).To enter, visit www.skincancer.org/school/teacherresources, follow the links to the Teacher Survey page, complete all required information and submit. Limit: One entry per person during the Promotion Period.
2. ELIGIBILITY: Promotion open only to legal US residents who are 18 years of age or older at time of entry and who are currently employed as a health or physical education teacher currently teaching children in grades 6-9 at an accredited school having a mailing address in the 50 United States or the District of Columbia. Employees of the Skin Care Foundation, CrossMedia Partnership (the “Sponsor”) and its parent and/or subsidiary companies, their advertising and/or promotion agencies, their respective parent companies, affiliates, or subsidiaries, and/or any other person or entity directly involved with this Promotion, as well as the immediate family members (parents, children, siblings, spouse) and members of the same households of these entities (whether related or not) are ineligible to participate in this Promotion.
3. PRIZES: Grand Prize: There is one (1) Grand Prize consisting of one (1) HD camcorder and one (1) MP3 player. Approximate Retail Value (“ARV”) of Grand Prize: $325.
First Prize: There are five (5) First Prizes of one (1) MP3 player. ARV of First Prize: $75 each.
Total ARV of all prizes available to be won: $700.
4. DRAWING AND NOTIFICATION: Grand Prize winner will be selected in a random drawing, from among all eligible entries received during the Promotion Period, on or about 05/11/09 under the supervision of the Independent Sweepstakes Administrator. Odds of winning will depend on the total number of eligible entries received. Winner will be notified by e-mail and/or telephone within two weeks of winner selection, and confirmed in writing by e-mail, US mail, or other form of delivery.
5. PRIZE CONDITIONS: No transfer or substitution of any prize is allowed, except at the sole discretion of the Sponsor. Winner is subject to any and all applicable rules, regulations and terms and conditions associated with the acceptance and use of any prize accepted. Grand Prize winner will be required to sign an Affidavit of Eligibility and Liability/Publicity Release to be returned within five (5) days of date of written notification, or prize award will be void and an alternate winner may be selected. If any prize or prize notification is returned as undeliverable, prize will be forfeited. All federal, state and other taxes are the sole responsibility of the winner. Any winner’s entry and acceptance of any prize constitutes permission for the Sponsor to use said winner’s name, photograph, likeness, statements, biographical information, voice and address (city and state) on a worldwide basis, and in all forms of media, in perpetuity, without notice or further compensation.
6. SWEEPSTAKES CONDITIONS: The Promotion is subject to all applicable federal, state and local laws. By participating, entrants agree to be bound by these Official Rules and the decisions of the Independent Sweepstakes Administrator and waive any right to claim ambiguity in regard to the mechanics of the Promotion or the interpretation of these Official Rules. Winner and entrants also agree to release, discharge, indemnify and hold harmless the Sponsor, its affiliates and their respective directors, officers, employees and agents, from and against any claims, damages or liability due to any injuries, damages or losses to any person (including death) or property of any kind, resulting in whole or in part, directly or indirectly, from acceptance, possession, use or misuse of any prize offered, or participation in the Promotion. Any person attempting to defraud or in any way tamper with the mechanics or logistics of the Promotion will be declared ineligible and may be prosecuted to the full extent of the law.

9. INDEPENDENT SWEEPSTAKES ADMINISTRATOR: The US Sweepstakes & Fulfillment Company, 625 Panorama Trail (Building #2, Suite 100), Rochester, NY 14625, which is responsible for overseeing the awarding of all prizes offered, and whose decisions shall be considered final in all matters related to this promotion.
10. The Sponsor retains the right to correct any and all printing, typographical or mechanical errors.
11. Sponsor is not responsible for malfunctions of electronic equipment, computer hardware, software, malfunctions of the entrant’s email provider, lost, late, incomplete, or misdirected entries, problems downloading entries from the website, lost or unavailable network connections, or failed, incorrect, inaccurate, incomplete, garbled or delayed electronic or other communications, whether caused by the sender or by any of the equipment or programming associated with or utilized in this Promotion which may limit the ability to participate, for any technical problems related to website entries, or caused by any human or technical error which may occur in the processing of the entries in this Promotion. Sponsor reserves the right to modify, suspend or terminate the Promotion in the event that it becomes infected by a computer virus or is otherwise technically impaired. In the event of a dispute regarding the identity of the person submitting an online entry, the entry will be deemed to be submitted by the natural person in whose name the e-mail account is registered. Normal Internet access and usage charges imposed by your online service will apply.
12. Caution: Any attempt by any person to deliberately damage any web-site, to be acting in violation of these Official Rules or in an unsportsmanlike or disruptive manner, or to undermine operation of a drawing is in violation of criminal and civil laws. Sponsor reserves the right to seek damages from such person to the full extent permitted by law.
